Charges and Insurance
Our fees reflect the value of the services provided. For all patients, payment in full is expected at the time of service. As a courtesy to you, and for your convenience, our office will bill your insurance company when all the requested information is provided. For your convenience we accept payments via cash, check, Visa, MasterCard, and Discover.
Most insurance policies cover visits to this office. Our staff will assist you in understanding your insurance and submitting claims. We do accept assignment on all Major Private Medical insurances and Medicare insurances. Several managed care plans such as HMOs, PPOs, EPOs, and POS Plans are also accepted. Feel free to call our office for details if your insurance is through one of the managed care plans. Any deductible amount, co-payment, or any other balance not paid for by your insurance company is your responsibility.
 
Appointments
Our office works by appointment because we recognize the value of your time and desire to serve you efficiently. Every effort is made to see our scheduled patients on time. Occasionally emergencies arise that may cause the delay of your appointment. We will make every effort to keep such delays to a minimum and will keep you informed of delays that occur.
If you are unable to keep your scheduled appointment, please call us at least 24 hours in advance. Also, if you know you are going to be late, please call. This courtesy allows us to accommodate other patients.
 
New Patients
On your first visit to our office, please plan to:
Come a few minutes early so you'll have time to fill out the necessary forms.
Bring a list of all medications you are presently taking, and any x-rays, medical records, referral forms, and diagnostic testing results that you have.
Bring your insurance card and a photo I.D.
 
Release of Medical Information
Information will not be provided to a third party (attorney, physician, insurance company, etc) unless we have a written authorization in our office. If you wish information about your condition to be provided to a third party, they should request the information in writing and provide us with a written authorization signed by you to do so. We will ensure that HIPAA requirements are met prior to release of medical information.
 
Prescription Refill
Prescriptions and refills can be requested by your pharmacist during our regular office hours. Have your pharmacy's phone number ready and call them at least two days before your prescription runs out. The pharmacist will obtain authorization from our physician.
